Veterinary Treatment
I have informed Top Cat Cattery of any medical condition that my cat/s suffers from and I am happy for these details to be checked with my vet. Please list details below.

I agree that in case of illness either

•	My Veterinary Surgeon listed on my booking form will be consulted and, if necessary, called in to carry out any treatment as he considers advisable
	and/or
•	Top Cat Cattery will consult with their own vet, who will be called in to carry out any treatment considered advisable

Veterinary Insurance
All cats staying at Top Cat Cattery are covered by our veterinary insurance.  The insurance covers

•	Vet fees to the sum of £300.00
•	Illness arising within 72 hours of the pets departure from Top Cat Cattery
•	Replacement value for the cat should it die by illness or accident whilst in the cattery

The insurance does not cover conditions that pre-existed your cat’s arrival at the cattery, gingivitis and dental conditions, and the cost of any necessary treatment.

These costs must be authorised to be paid at your own expense or by your own medical insurance if they are not covered.  The account for any treatment is payable to Top Cat Cattery on collection of your cat(s).

Flea Treatment
If my cat(s) is discovered to have fleas during its stay at Top Cat Cattery I understand that it will be treated with Frontline flea spray or neck drops and the cost, which covers the cat’s treatment and spraying of the run, will be paid on collection.

This agreement is ongoing and covers every subsequent stay (medical details updated as necessary).
Whilst every care and precaution is taken, responsibility can only be accepted at owner’s risk.
